ENROLLMENT REQUIREMENTS: 1. Only parents or legal custodians may enroll students.
A copy of custody papers MUST be provided, if applicable.
2. Proof of Domicile (2) within the China Grove Middle attendance area. Examples of proof are a rent/lease agreement, sales contract, power bill, telephone bill, etc.
3. Withdrawal form from student's previous school. If no form is provided, enrollment will be delayed until the previous school can be contacted.
4. Copy of report card or transcript from previous school.
5. Copy of Birth Certificate.
6. Copy of Social Security Card.
7. Immunization Records.
8. Written documentation of EC status, if applicable.
